Karadja rugs are handknotted with a stylised repeat format and stylised medallions as seen with 3 here.  The older the rug the more motifs fill the field surrounding them and here its relatively busy, which is one of the giveaways that this is circa mid 1900s.  The quarter star corner spandrels if joined would create another of the central medallion motifs.  Karadja rugs and runners are always filled with huge energy and movement.
